Our Team
Dr. Damen Patel
About Dr. Patel
Dr. Patel graduated from the State University of New York in Buffalo, School of Dental Medicine in 1995. Upon graduation, he completed a one-year general practice residency program at Waterbury Hospital in Waterbury, Connecticut. He then worked at a dental clinic and was in a private practice in New York State. In 2002, Dr. Patel and his family had the opportunity to move to Sidney, join Dr. Edelen’s practice and also be closer to family. Dr. Patel and his wife are proud parents of two daughters and a son He is a self-proclaimed golf addict. He loves to play, read and dream about golf. As a dentist, he believes in treating patients as he would family, and offering them the latest that technology has to offer in order to help them achieve dental health. Dr. Patel has taken continuing education courses with the US Dental Institute to offer both adult and children patients orthodontic care. Through an externship program with The Midwest Implant Institute Dr. Patel is certified in surgically placing dental implants. With decades of experience in the field, Dr. Patel specializes in orthodontics and dental implants, helping patients achieve healthy, confident smiles through advanced and personalized care. As part of his dedication to dental wellness, Dr. Patel has traveled on mission trips with a dental team to provide dental care in remote villages of Tanzania. In Miami and Shelby county, Dr. Patel and his staff also treat children that are in need of dental care through the Give Kids a Smile program. Dr. Patel and his wife are proud parents Mira, Riya, and Rohan.
Dr. Nicole Lochard
⬇️ About Dr. Lochard
Dr. Nicole Lochard returned to her hometown of Sidney, Ohio to join MidWest Ohio Dental Care upon graduation from The Ohio State University College of Dentistry in May of 2019. She attended The Ohio State University for both undergraduate and dental school. As an undergraduate, she majored in Biomedical Sciences, which is a highly selective program focusing on undergraduate medical research. While there for her undergraduate work, she completed 3 years of research in Huntington's Disease and co-founded a research and education organization for Multiple Myeloma. Dr. Lochard graduated as one of the top ten students of her dental school class, earning honors and being admitted into Omicron Kappa Upsilon Dental honors fraternity. Dr. Lochard is married to Alex Lochard, also a native of Sidney. They currently have two children, Reese and Austin. Their family resides in Sidney and they love being very involved in the community that they grew up in. Dr. Lochard is a past president and current member of the Western Ohio Dental Society, a member of Shelby County Charity League and Holy Angels Catholic Church. Dr. Lochard performs general dental care, clear aligner orthodontics, and Botox. She is a member of The American Dental Association and Ohio Dental Association. She enjoys serving her hometown by providing high quality, modern dental care.
Dr. Rebekah Slagle
⬇️ About Dr. Slagle
Dr. Rebekah Slagle is a proud 2025 graduate of The Ohio State University College of Dentistry and is thrilled to be joining the Midwest Ohio Dental team. She completed her undergraduate studies in Biology and Chemistry at the University of the Cumberlands in 2019, where she was also an athlete on the collegiate volleyball team. Dr. Slagle further advanced her education with a Master's in Business Administration from the University of Northwestern Ohio in 2021, during which she also served as a Graduate Assistant within the volleyball program. Born and raised a Buckeye, it was a natural decision for Dr. Slagle to continue her academic journey at Ohio State, where she developed a strong foundation in dentistry. Her passion for the field was ignited early in life, inspired by the exceptional care provided by her childhood orthodontist. This early experience sparked her dedication to improving the lives of others through dental care. Dr. Slagle is committed to expanding her expertise in general dentistry, while continuing to learn from experienced mentors and advancing her knowledge in the areas of orthodontics and implant surgery. She is eager to grow professionally and to provide top-tier dental services to her patients. Originally from Delphos, Ohio, Dr. Slagle was introduced to Sidney by her husband, Jon, a native of the area. The couple are active members of the Holy Angels Catholic Church and excited to make Sidney home for their future family. She looks forward to becoming a vital member of the community while fostering lasting relationships with her patients. Outside of the office, Dr. Slagle maintains a strong passion for health and fitness. She is often found at the local YMCA, crossfit gym, or running around town. In her leisure time, she can be caught online shopping or taking walks with Jon and their golden retriever pup, Olie. Dr. Slagle is a member of the American Dental Association, Ohio Dental Association, and the Western Dental Society. She is looking forward to contributing to the well-being of her patients and becoming an integral part of the Sidney community.

I was born and raised in Sidney and graduated from Sidney High School in 2010. I have been married to my husband Nathan since 2020 and we currently live in Anna with our two daughters Aleigha and Ariana.
What is the best part of your job?
All my co-workers that have become more like family and helping to create the best environment for all our patients.
What do you like to do for fun?
We enjoy 4-wheeling in our RZR and spending time with family and friends.
Why did you decide on a career in dentistry?
I have always been interested in health care. The first thing I have always noticed are people's smiles!

Where do you hope to travel one day?
Anyplace warm and sunny on the water.
What is the best part of your job?
All my co-workers and meeting new people.
What do you like to do for fun?
I enjoy camping and boating with my family, horseback riding, and golf.
Why did you decide on a career in dentistry?
I've had an interest in teeth since I was a child. I attended Miami Valley Career Technology Center my junior and senior year of high school and have been working in the field since then. I love working with people to improve their smile and health.

Hi, I'm Danielle! I'm almost 31 and a proud mom of two — a daughter and a son. I live out in the country on our family farm in Sidney.
In my free time I love cooking, baking, all things truck related, and watching the Cincinnati Bengals. Family, farm life, and simple joys keep me grounded and smiling every day.
Where do you hope to travel one day?
One day I'd love to travel to Maine for its beautiful coast, explore the history and charm of Salem, Massachusetts, and take in the mountains of Colorado.
What is the best part of your job?
The best part of my job is the people — working alongside supportive co-workers in a positive environment, building connections with patients, and being part of a field that truly helps others every day.
What do you like to do for fun?
In my free time, I enjoy spending time with my kids, baking and trying out new dinner recipes, and working outside in my flower bed.
Why did you decide on a career in dentistry?
I chose a career in dentistry because I've always been drawn to a field where I can make a difference in people's lives. I enjoy being part of a profession that not only provides care but also comfort and confidence for patients.

About Priti
I am Dr. Damen Patel's wife. I grew up in Columbus, Ohio and earned my MBA from Mount Saint Mary's College in New York.
I have been working at our office since 2005. I help keep the office running smoothly by managing the bookkeeping and handling all the behind-the-scenes details.
Outside the office, I am a mom to three wonderful kids, Mira, Riya and Rohan. I enjoy spending time with my family and creating wonderful memories together.
I also volunteer with various local organizations in the community and give back whenever I can.

I was raised in Minster and graduated from Minster High School in 2003. I joined Midwest Ohio Dental Care in June 2020.
I have been married to my husband Paul since 2011 and currently live in Anna with our son Gabriel and our dog Bolt.
Where do you hope to travel one day?
I would love to get a tiny camper and travel across America.
What is the best part of your job?
I love helping people and seeing them smile when they are done with their appointment and seeing them again in six months.
I also really enjoy being around my co-workers, they are like my family.
What do you like to do for fun?
I love spending time with my friends and family. We really enjoy sitting around the fire almost all year round, fishing, watching movies, and going on cruises.
Why did you decide on a career in dentistry?
I didn't choose it, it chose me. My first job in dentistry I found fascinating and enjoyed seeing all the smiley faces.

I'm from Anna and have one younger brother. My dad's side of the family is from Pennsylvania, so we travel there throughout the year.
Where do you hope to travel one day?
I hope to travel to all 50 states and to Italy.
What is the best part of your job?
The best part of my job is seeing the before and after the cleaning. I also want to make sure every single one of my patients has the best smile that they can have!
What do you like to do for fun?
I like to hang out with my friends and family. I also like to go hunting and ride four wheelers.
Why did you decide on a career in dentistry?
I wanted a career in healthcare. I looked into dentistry and was automatically drawn in!

I have worked in the dental field since 2011. I attended the Dental Assistant Training School (DATS, LLC) in Troy, Ohio. During that time, I obtained my Dental Radiographer's License from the Ohio State Dental Board.
I enjoy learning from my peers and taking continuing education courses to stay up-to-date on dental healthcare.
Aside from assisting, I am also a wife and mother. My husband and I have been married since 2013 and have been blessed with four children.
Where do you hope to travel one day?
As a family, I would love to travel to Niagara Falls. We love to hike to admire waterfalls and explore nature.
We've also talked about returning to Lake Tahoe one day.
What is the best part of your job?
I look forward to meeting new and familiar faces while ensuring their dental experience is something to look forward to rather than fear.
What do you like to do for fun?
I enjoy spending time with my family, being outdoors, and cheering our children on from the sidelines of their sporting events.
Why did you decide on a career in dentistry?
I chose a career in dentistry because I've always looked forward to dental visits. I was at the dentist every month during my teenage years for orthodontic treatment, and the staff were always so welcoming.

I have been at Midwest Ohio Dental Care since 2006. Since starting here, I have received my EFDA license and have training in implants and phlebotomy.
I was born and raised in Piqua, Ohio, and became a traitor in 2018 and moved to Troy. GO TROJANS!
I have two amazing children, one son and one daughter — the best of both worlds.
Where do you hope to travel one day?
What is the best part of your job?
I like all the up-to-date technology that we offer in office, including CAD/CAM, 3-D imaging, and 3-D printing.
What do you like to do for fun?
I enjoy making memories with my kids! We spend time at the pool and amusement parks all summer long.
Why did you decide on a career in dentistry?
I chose dentistry because it seemed like a good fit. Healthcare was intriguing, but I didn't want to be a nurse. Plus, I LOVE smiles!
